This form serves as the primary method for requesting a new or replacement PAR Tag. The PAR Tag system is a crucial element to maintaining accountability and safety on Chatham Fire scenes. All Fire, EMS, and Volunteers should turn their tag into command whenever arriving at any Chatham Fire scene. Requests are typically processed within 1 week and will be delivered to your primary station. It is recommended that you provide an email address for easier communication with PAR Tag Team.
